OSRS Maggot King Guide: Gear, Phases, Melee Punish and Drops
OSRS Maggot King guide: requirements, phase HP table, gear by budget, the 4- and 5-tick melee punish, larvae, drop rates and all 9 Combat Achievements.

In this guide 12 sections
Quick summary
- Requirements: finish The Blood Moon Rises, then reach Vampyrium through Castle Drakan. The boss is solo-only, in an instance east of Sangvesti.
- Bring two styles: fire spells or heavy ranged (enchanted ruby bolts) for the first 35%, plus a crush melee weapon for the melee phase from about 980 HP (65%). You also need a fast standard bow to shoot larvae before they land.
- Turn off your protection prayer before every screech. If you don't, you take 15 to 25 damage per tick.
- The melee punish decides your kill speed. Click the boss on the tick it extends its head for +50% damage and much higher crush accuracy.
- Drops: any unique is 1/205.6 (Elder venator fang 1/340, Crimson kisten 1/520). The pet is 1/3,500 from Open-stomach or about 1/1,502 from Take-eggs (Jagex rates via the OSRS Wiki).
This OSRS Maggot King guide covers the solo boss Jagex added to Vampyrium on 30 June 2026 with The Blood Moon Rises. Numbers come from the OSRS Wiki's Maggot King page and its strategy page, which carry Jagex's published drop rates. It applies to Old School RuneScape only.
Maggot King requirements
| Requirement | Level or detail | Why |
|---|---|---|
| The Blood Moon Rises | Quest complete | Unlocks Vampyrium |
| Ranged (if ranging) | 85+ recommended (wiki); 80 minimum (Tpose) | Phase 1 damage |
| Magic (if maging) | 75+ recommended; 95+ for fire surge setups (Tpose) | Phase 1 damage |
| Attack / Strength | 80+ / 85+ | Melee phase and punishes |
| Prayer and Defence | 74 Prayer, 70 Defence for Rigour | Strongly recommended |
| Construction | 70 (boostable) | Bank chest in Sangvesti |
| Agility | 64 | Two bush shortcuts in Sotfa Forest |
The Blood Moon Rises itself needs 74 Slayer, 74 Woodcutting, 72 Smithing, 72 Cooking, 70 Fletching, 66 Mining, 65 Hunter, 64 Crafting, 64 Herblore and 57 Magic (none boostable), plus A Night at the Theatre and Sins of the Father. If the skills are your bottleneck, the OSRS Woodcutting guide, OSRS Smithing guide and OSRS Hunter guide cover three of the requirements that most often hold accounts back.
How to get to the Maggot King
- Teleport with Drakan's medallion to Castle Drakan (unlock the teleport by inspecting the shrine by the castle's front door during the quest). Then enter the Vampyrium portal.
- Rescue the aranei scout south-east of Sangvesti. Kill the three sanguidae attacking it, then talk to it. From then on it waits south of the bank chest, by the transportation map icon, and takes you just east of Sangvesti.
- Build the bank chest in Sangvesti at 70 Construction with a hammer, saw, 1 magic stone, 2 cupronickel bars, 2 adamantite bars and 4 ironwood planks.
- Use the far bush shortcut (64 Agility). The wiki says the closer one is a longer path and tends to pull an Ancient feral vyre.
- Enter through the darkwood trees at the lair. A scoreboard outside tracks your personal and global records.
If you die: you respawn by the Vampyrium portal and your grave appears outside the lair. Death fees are cut by 75% until you have 5 kills. The aranei scout near the portal will move your grave for 50,000 coins, 1 vial of blood or 150 stymphike feathers (wiki).
Maggot King mechanics, phase by phase
The Maggot King has 1,500 HP, is combat level 741 and is 5x5. Its magic and ranged hits reach 46 and are fully blocked by the matching protection prayer. The melee lunge reaches about 55, and Protect from Melee only cuts it to about 35.
| Boss HP | What changes | Your action | Prayer |
|---|---|---|---|
| 1,500 to ~1,250 | Ranged spits (poison bile), 4 to 8 attacks, then a screech | Fire spells or ruby bolts from your side | Protect from Missiles, off for screech |
| After the 1st screech | Carrion falls; the boss switches to magic and attacks 1 tick faster | Dodge large carrion shadows, tank small bits if needed | Protect from Magic, off for screech |
| ~1,250 (83%) | Larvae launch with every screech, west first, then alternating | Shoot each larva with a standard bow before it lands | As above |
| ~980 (65%) | Melee slams after each screech; magic defence rises to +400 during melee | Switch to crush melee and punish every slam | Protect from Melee (reduces to ~35) |
| ~305 (20%) | It stops retreating and only uses melee | Ignore screech prayer flicks and keep punishing | Protect from Melee |
| ~299 (19%) | Up to 4 larvae at once | Use a Venator bow (bounces) or fast bow clicks | Protect from Melee |
Numbers are from the OSRS Wiki strategy page and are approximate thresholds.
Handle the bile and carrion
- Poisonous bile (green, from ranged attacks) poisons you, drains poison immunity and does chip damage. A Serpentine helm plus Anti-venom+ makes you immune.
- Sticky bile (yellow, from magic attacks) drains Attack, Strength or Defence and delays your next attack by a tick. An Inoculation bracelet makes you immune.
- Carrion lands during each screech and wipes bile off the tiles it hits. Large piles hit for up to 25 and small bits for up to 10. Go for the most isolated shadow, because overlapping small bits can hit "safe" tiles.
Example: at 1,100 HP the boss screeches. You flick Protect from Magic off, step to an isolated shadow-free tile, shoot the larva on the west side with your Webweaver bow, then turn prayer back on as the magic spits resume.
Shoot the larvae
Ur-maggot larvae die instantly to standard ranged (bow and arrows) while still in the air. If one lands, it walks along its row, eats carrion to heal the boss 1 HP, and then latches onto you for 3 damage, growing by 1 per tick, which also heals the boss.
The wiki notes a useful trick: a bow shot fired right after a slow attack overwrites the slow weapon's cooldown. With a 3-tick or 4-tick bow, this lets you fit a second slow melee hit (even with a Dharok's greataxe) before the first slam.
How to do the Maggot King melee punish
From ~980 HP, after each screech the boss walks to the bone wall, lines its centre tile up with yours and slams a 3x4 area. It also throws three bile pools within two tiles of the slam.
- Dodge sideways to its easternmost or westernmost tiles as it winds up.
- Click the boss on the tick it extends its head. That attack fires with no attack delay, +50% damage and with crush, an 85% reduced defence roll. On multi-hit weapons only the first hit gets the bonus.
- Attack again for your regular hit.
| Method | Weapon speed | How it works | Watch for |
|---|---|---|---|
| Stepback | 4-tick (e.g. Inquisitor's mace) | Step back a tile so the boss's movement doesn't cost you ticks | The stepback tile may have bile on the first slam |
| Drag | 5-tick (Scythe of Vitur, Soulreaper axe) | Move east or west; no ticks lost | Don't drag into bile |
| Punish switch | Any main plus a slow punish weapon | Punish with Elder maul, Dharok's or Colossal blade, then switch back | Extra clicks every slam |
| Dharok's | 7-tick, at 1 to 15 HP | A drag-drag-stepback cycle every 3 slams | Advanced only. Eat low-heal food (Guthix rest) to stay at 20 to 30 HP between phases |
The wiki favours damage special attacks over defence-reducing ones, because the boss resets its defence after each melee phase. If you do use a Dragon warhammer or Elder maul spec, use it on a punish.
Best gear for the Maggot King by budget
| Slot or role | Max | Mid | Budget |
|---|---|---|---|
| Magic weapon | Harmonised Nightmare staff (fire surge) | Twinflame staff (fire wave) | Purging staff, Kodai or Toxic staff with fire surge |
| Magic offhand and gloves | Tome of fire, Confliction gauntlets | Elidinis' ward (f) | Mage's book or Malediction ward, Tormented bracelet |
| Ranged weapon | Zaryte crossbow | Dragon hunter or Armadyl crossbow | Hunters' sunlight crossbow (wiki: better than Twisted bow here), Rune crossbow |
| Ranged ammo | Ruby dragon bolts (e) | Ruby bolts (e) | Moonlight antler bolts |
| Larva bow | Webweaver bow, Bow of Faerdhinen | Venator bow (multi-larvae) | Magic comp bow, Crystal bow |
| Melee main | Obligator, Scythe of Vitur (crush) | Soulreaper axe (crush), Inquisitor's mace (4-tick) | Abyssal bludgeon, Zombie axe |
| Punish weapon | Elder maul, Colossal blade | Bandos godsword | Barrelchest anchor, Dharok's greataxe |
| Specials | Crimson kisten, Eye of Ayak (magic) | Voidwaker, Dragon claws | Dragon warhammer |
Pack this inventory (wiki magic setup)
Melee switch (Scythe, Inquisitor's helm, hauberk and plateskirt, Amulet of rancour, Infernal cape, Ferocious gloves, Ultor ring), Eye of Ayak, Webweaver bow, 10 Anglerfish, 4 Sanfew serums, 1 Surge potion, 1 Divine super combat, a Saturated heart, a rune pouch (wrath, air, fire) and Drakan's medallion. The ranged version swaps the Ayak for a Zaryte crossbow, the heart for a Divine ranging potion and adds Book of the Dead.
Tip: the boss often drops a Dull ancient medal and 2 to 4 Stymphike tartare together. They restore prayer and heal, and often add a kill to a trip.
Maggot King drops: Open-stomach or Take-eggs
After each kill you loot the corpse one of two ways:
| Option | What you get | Pet chance |
|---|---|---|
| Open-stomach | Normal table: uniques 1/205.6 (Elder venator fang 1/340, Crimson kisten 1/520), resources, supplies, tarnished items | 1/3,500 |
| Take-eggs | Usually nothing, sometimes supplies, or one of six maggot eggs that each give a pet roll when popped | About 1/1,502.4 total |
Both options can drop an elite clue (1/40, or 1/38 with elite Combat Achievement rewards). On a Konar task, Brimstone keys drop at 1/50 from Open-stomach. Choose Take-eggs only if the pet is your goal: it gives up every unique.
Value note: the wiki's money-making guides estimate about 12.1M gp/hour for melee and magic and about 9.9M for ranged. Most of that comes from the two uniques, so your real rate swings with luck and Grand Exchange prices. The fang becomes tradeable once etched, and at 84 Crafting it upgrades the Necklace of anguish into the Necklace of rupture, the strongest ranged necklace in the game. See the OSRS money making guide for comparison methods.
Maggot King Combat Achievements, in order
| Order | Task | Tier | What to do |
|---|---|---|---|
| 1 | Maggot Squasher: 1 kill | Hard | First kill |
| 2 | Maggot Exterminator: 25 kills | Elite | Comes with normal grinding |
| 3 | Trying to fit in: only Vampyrium weapons | Elite | Plan a Vampyrium-weapon kit, such as the Venator bow and Crimson kisten |
| 4 | King-sized clobbering: no missed melee punish | Master | Once punishing is automatic |
| 5 | Digging in: 40 tiles of movement or fewer | Master | Minimise dodging; tank small carrion |
| 6 | Cordoned Off: stay on the back row | Master | Pairs naturally with Digging in |
| 7 | Perfect Maggot King: no avoidable damage, no healing | Master | Kill every larva |
| 8 | Camping the King: 12 kills without leaving | Master | Bring max sustain and loot tartare |
| 9 | Speed Chaser: first 5 kills within 9 minutes | Grandmaster | Max gear and the 5-tick drag |
There are 9 tasks worth 42 points in total (wiki).
Common Maggot King mistakes
- Leaving a protection prayer on through the screech. Every tick of the screech hurts while an overhead is active, so turning prayer off is the habit to build on kill one.
- Ignoring a larva because the boss is low. A landed larva heals the boss twice: once from carrion, then again while it is latched on you.
- Bringing only one combat style. The melee phase from about 65% is not optional, and a magic or ranged weapon alone cannot finish the kill.
- Spending defence-reduction specs early. The boss resets its defence after each melee phase, so damage specials on a punish usually do more.
- Dodging more than you need to. Small carrion bits hit for up to 10, and the Master tasks reward standing still, so tank them when your HP allows.
Example: your first kills keep stalling with the boss healing near the end. The usual cause is a larva landing during the multi-larva phase from about 19%. Switch to a Venator bow before that threshold, or carry a faster bow, and the healing stops.
If you're returning, boosted, or on an alt
- Returning players: Vampyrium, the Maggot King and The Blood Moon Rises are all new in 2026. Start with the quest, then the scout and bank chest unlocks above. Since July 8, loot goes straight to your inventory, and ammo always lands inside the arena.
- Boosted accounts: if someone else trained your stats, practise the punish timing before you rely on it. Missed punishes cost you more here than missing gear upgrades. Your first five kills carry a 75% death-fee reduction, so use them to learn.
- Alts and Ironmen: the supply drops and resource rolls (runite ore, Orikalkum gravel, Venator fangs and teeth) help Ironmen. For a Slayer alt, the boss can be assigned by Konar, Nieve and Duradel; see the OSRS Slayer guide. If you haven't done a prayer-switching fight yet, the Fire Cape guide and the OSRS Zulrah guide are good primers, and the OSRS Vorkath guide covers a common earlier money boss.
The Maggot King is one stop on a longer PvM path, and the Old School RuneScape hub lists the guides and services around it.

Frequently asked questions
How do I get to the Maggot King in OSRS? Finish The Blood Moon Rises, teleport to Castle Drakan with Drakan's medallion, enter Vampyrium and rescue the aranei scout south-east of Sangvesti. The scout then takes you close to the lair east of Sangvesti.
Can you kill the Maggot King with only ranged or only magic? No. Melee is required: from about 980 HP the boss slams after every screech, and from about 305 HP it only uses melee. You need a ranged or magic style for the start and a crush melee weapon for the finish.
What prayer do you use against the Maggot King? Protect from Missiles during the ranged rotation and Protect from Magic during the magic rotation. Turn prayer off for every screech, and use Protect from Melee in the melee phase.
What does the Maggot King drop? Any unique is 1/205.6: the Elder venator fang (1/340) and the Crimson kisten (1/520). The fang, etched at 84 Crafting, upgrades a Necklace of anguish into a Necklace of rupture. Take-eggs swaps all normal loot for a better pet chance (about 1/1,502 instead of 1/3,500).
Is the Maggot King worth it for money? The wiki's money-making pages estimate about 12.1M gp/hour with melee and magic, and about 9.9M with ranged. Most of that value is the two uniques, so short sessions can land well below it.
Is the Maggot King a Slayer task? Yes. Konar, Nieve and Duradel can assign it, it gives 3,675 Slayer XP, and an imbued Slayer helmet works on task.
