Quick summary
- What it is: a three-boss poison dungeon added in Patch 12.1 and part of the Season 2 Mythic+ pool. The bosses are Rav'i, The Writhing Coil, and Zul'jan (Method, TheWoWDB).
- The three checks: move Rav'i to the clean bone pile before she feeds. Kick all three of the Coil's casts in a row. Clear Ritual Venom on Zul'jan before it runs out (TheWoWDB, Method).
- Gates: destroy six Caustic Mist Totems to reach Rav'i. Then break four Infusion Totems and kill the Ascendant Serpent to reach Zul'jan (Method).
- Free buff: anyone with 25+ Midnight Cooking or Alchemy can use the Unfinished Mixture after the second boss to buff the group.
- Rewards: The Writhing Brood mount can drop from Zul'jan on Mythic and Mythic+. Time a +10 for the dungeon's teleport (TheWoWDB).
Find the entrance and plan the run
Altar of Fangs is inside the Vaults of Atal'Utek on the Coiled Isle. Our Coiled Isle guide covers the trip there.
| Fact | Value | Source |
|---|---|---|
| Bosses | 3 (Rav'i, The Writhing Coil, Zul'jan) | Method, TheWoWDB |
| Difficulties | Normal, Heroic, Mythic, Mythic+ | Icy Veins |
| Mythic+ timer | 30:00 | Wowhead |
| Gates | 6 Caustic Mist Totems; 4 Infusion Totems + Ascendant Serpent | Method |
| Profession buff | 25 Midnight Cooking or Alchemy | Method |
| Teleport | Time it at +10 or higher | Blizzard via our M+ guide |
Clear the opening area and the totems
The six Caustic Mist Totems are guarded by these packs. TheWoWDB says not to skip past them; plan your first pulls around them.
| Mob | What to do | Source |
|---|---|---|
| Venom Leech | Gorge heals it on melee hits. Step out of the Septic Spatter circles when it dies | Method |
| Twinfang Harrower | Can't be CC'd. Face Toxic Breath away from the group. Dispel Paralyzing Shots with a magic dispel, freedom, or an immunity | Method, Icy Veins |
| Ravenous Descendant | Its Ravenous enrage stacks. Soothe it or CC it to reset the stacks | Method |
| Primal Serpent | Kick every Piercing Hiss, or the group's Haste drops | Method |
| Ritual Chieftain | Can't be CC'd. Unstable Totem pulses group damage until it dies. Blood Sacrifice hits the group and applies a heal absorb. Dismember hits the tank | Icy Veins, Method |
Icy Veins calls the Ritual Chieftain "the most important mob in this area," so kill it first.
Beat Rav'i: steer her to the clean pile
Rav'i is a hydra in an arena with three bone piles (Carrion Piles).
- Ravenous Stomp hits the whole group and drops falling rocks to dodge. It also throws Twinfang corpses (Fresh Meat) onto two of the piles (TheWoWDB).
- Ssscavenging: when her Energy runs out, she walks to the nearest pile and eats behind an absorb shield.
- Feeding Frenzy: since the August 21 hotfix it no longer speeds up Messy Eater or Carrion Burst, but her shield still has to be burned (Blizzard).
- Scent of Blood: she gets this buff near Fresh Meat corpses, and it makes her shield bigger (TheWoWDB, Icy Veins).
- Messy Eater: puddles land while she eats.
- Triple Shot hurts allies within 5 yards of the targets. Regurgitate sends acid waves that cut movement and damage by 50%. Method says a disease dispel removes it. Hydrastrike is the heavy tank hit (Method).
What to do: after each Stomp, the tank finds the pile with no corpse on it and drags Rav'i there before her Energy hits zero. DPS pour everything into the shield. Everyone soaks the puddles. Healers save a cooldown for each feeding phase. TheWoWDB calls the feeding phase "a soft enrage for your group."
Handle the trash before The Writhing Coil
TheWoWDB reports that an August 25 hotfix removed one High Evolutionist from this part of the dungeon.
| Mob | What to do | Source |
|---|---|---|
| High Evolutionist | Kick Envenom and dispel the poison. Stop the Evolve channel with CC; if it finishes, Envenom becomes Mass Envenom, which you should kick | Method, Icy Veins |
| Rattling Writhe | Can't be CC'd. Rattle is a channel that hits the whole group, so plan defensives. Corrosive Fangs hits the tank | Method, Icy Veins |
| Bloodletter | Its melee hits leave puddles. Keep moving the pack | Method |
| Hatchling | Hatches from eggs you step on and fixates a player. Slow, root, or stun it, and don't scatter | Method |
Beat The Writhing Coil: kick, snap, cleave
- Three kicks in a row: the boss tries three casts back to back. Assign at least three interrupters before the pull (TheWoWDB).
- Synchronized Venom deals ticking group damage for most of the fight (Method).
- Vindictive Onslaught: sidestep the Burrowing Charge line, then aim the Venom Jet cone away from the group.
- Death Rattle + Vine Grip: group damage that keeps growing until everyone stretches and snaps their tether. Use movement abilities (Method, TheWoWDB).
- Uncoil: the boss splits into five Uncoiled Writhes. Whatever health they have left becomes the boss's health, so damage on them counts. Three of them cast Toxic Atrophy. Writhes on Spiteful Hunt should be CC'd before they reach their target (Method, Icy Veins).
- Recoil: about 25 seconds in, the Writhes regroup in the middle of the room. That makes Undermining shockwave lines, and Assimilation, which Icy Veins says "might turn lethal." Stay out of where they merge.
What to do: stack the Writhes, CC them, and use your burst cooldowns during the split.
Take the Unfinished Mixture and kill the Ascendant Serpent
The Unfinished Mixture is in the back-left corner of the room after the second boss. A player with 25+ Midnight Cooking or Alchemy can buff the group for the rest of the dungeon. Then break the four Infusion Totems to free the Ascendant Serpent. It can't be CC'd (Method).
- Infest: spread out, then group up so you can cleave the Hatchlings it spawns.
- Virulent Whirl: avoid the moving twisters.
- Noxious Spray: a frontal that pushes the tank back. Face it away from the group.
Last trash before Zul'jan: Blade of the Altar (Laced Edge damage over time), Ula'tek's Chosen (kick every Mass Envenom, dodge the Toxic Surge lines), and Living Venom. Living Venom explodes when it dies, so stagger the kills.
Beat Zul'jan: soak, then cleanse with physical hits
- Ritual of the Fang: four beams travel toward Zul'jan for 8 seconds. Any beam that reaches him gives him Fang Empowered, which deals stacking group damage. Players have to block the beams (TheWoWDB).
- Ritual Venom: blocking a beam gives you this debuff for 50 seconds. It explodes when it runs out, and it stacks on Mythic and Mythic+ (citing the Journal).
- Bloodletting: physical damage removes your Ritual Venom stacks, but each removal drops a blood pool that lasts about 30 seconds (Method, TheWoWDB).
- Boneslicer: an axe throw in a line that clears up to 8 stacks between the hit and its 7-second DoT (Method).
- Axegrinder throws three bouncing axes. Chop Down is a two-hit tank combo that also drops pools if the tank has stacks.
| Group type | How to clear Ritual Venom | Source |
|---|---|---|
| Backup | A deliberate Axegrinder hit also clears stacks, but it's harder to control | TheWoWDB citing Icy Veins |
What to do: assign beams before the pull and send each player back to the same beam every Ritual. Drop your blood pools at the edge of the room and keep the X-shaped beam lines clear. Rotate who soaks so no one builds too many stacks.
Compare the routes
| Option | Strengths | Weak points | Source |
|---|---|---|---|
| Method MDT PUG route (wago.io/qak6G3Uvs) | Import it once and everyone sees the same pulls | Needs the Mythic Dungeon Tools addon | Method |
| Full clear | Easy to learn on Mythic 0 | Wastes time on the timer | Our assessment |
Assign interrupts and stops
| Cast or mob | Answer | Source |
|---|---|---|
| Piercing Hiss (Primal Serpent) | Kick every cast | Method |
| Coil's triple cast | Three assigned kicks | TheWoWDB |
| Evolve (High Evolutionist) | Stop it with CC | Method |
| Mass Envenom (Ula'tek's Chosen, evolved High Evolutionist) | Kick every cast | Method, Icy Veins |
| Twinfang Harrower, Ritual Chieftain, Rattling Writhe, Ascendant Serpent | Can't be CC'd. Use defensives and dispels instead | Method, TheWoWDB |
Our interrupt coordination guide shows how to set a kick order.
Know the loot, mount, and hotfixes
TheWoWDB's datamine lists 7 Journal drops for Rav'i, 8 for The Writhing Coil, and 12 for Zul'jan, including the mount. The four trinkets are Coiled Fangstone and Vile Vial of Volatile Venom (Rav'i), Knot of Writhing Serpents (the Coil), and Tattered Amani War Banner (Zul'jan) (TheWoWDB). Item levels depend on key level; see our Season 2 Mythic+ guide. It lists +10 as the cap: 311 at the end of a run, 318 in the Vault. The mount is covered in our Season 2 mounts guide, and set pieces in our tier set guide.
| Date (2026) | Hotfix | Source |
|---|---|---|
| August 14 | Knot of Writhing Serpents no longer drops for healer specs | TheWoWDB |
| August 21 | Ravenous Descendant gains half as much attack speed per stack | TheWoWDB |
| August 25 | One High Evolutionist removed from the trash before the Coil | TheWoWDB |
Achievements: Altar of Fangs, Heroic: Altar of Fangs, Mythic: Altar of Fangs, and Keystone Hero: Altar of Fangs. Track them on our achievements page.
Run it on alts or after a break
- Returning for 12.1.5: our Season 2 Mythic+ guide found no dungeon tuning in the notes, so the August hotfixes above are still the latest changes we know of. Older videos may show the removed High Evolutionist and the stronger Ravenous Descendants.
- Alts: get a crafter alt to 25 Cooking or Alchemy so your group always has the Mixture.
- Mount farming: The Writhing Brood is a chance drop on Mythic and Mythic+ only (TheWoWDB).
